Your hospice team, which includes your doctor, will collaborate with you and your family in developing a plan of care based on your needs. Your care plan will then include Medicare-covered hospice support and care.
If you are eligible for hospice care, a special care team or support staff will be assigned to you, facilitating care that helps you and your family manage your illness.
Aside from you and your family, the other people on the team may include:
- Doctors
- Nurses or Nurse Practitioners
- Counselors
- Social Workers
- Hospice aides
- Homemakers
- Volunteers
- Spiritual in Counselors
- Bereavement Counselors
Additionally, to give you and your family support and care whenever you need them, a hospice nurse and doctor will be on-call 24 hours a day, 7 days a week.
It’s best to note that a hospice doctor is included in your medical team. Your primary care physician or staff can act as the attending medical professionals in your new medical team to supervise your care.
The hospice benefit allows you and your family to stay together in the comforts of your home unless you need urgent care in an inpatient facility. If the hospice team determines that you need inpatient care, they can make the arrangements for your stay.
